What we do
Certified billers handle every claim from charge capture to payment posting. We file both professional (CMS-1500) and institutional (UB-04) claims, and we work each one until it is paid, not just until it is submitted.
We work with small and mid-size practices across the U.S.: solo physicians, nurse practitioner clinics, physical therapy and chiropractic offices, behavioral health groups and multi-provider practices. You keep your EHR and your front desk; we take everything from the charge to the deposit.

How we run your billing
Eligibility and authorizations
Coverage, benefits and authorization rules are checked before the visit, so the claim starts clean.
Charge capture and coding review
Charges are checked against documentation before anything reaches a payer.
Claim scrubbing
Every claim is validated against payer rules, so avoidable rejections never leave our desk.
Error correction and timely filing
Issues are fixed and filed inside each payer’s deadline, not after it.
Payer follow-up
Unpaid and underpaid claims get chased until they are resolved.
Payment posting and reporting
Payments are reconciled against every ERA and reported back to you in real time.
What medical billing costs
One rate for the full revenue cycle, not billing alone: you pay a share of what we collect, and the share gets smaller as your collections grow.
Full-service rate
3% to 6%of monthly collections, set by your volume. It covers every step listed below. The more you collect, the lower your percentage.
Setup and onboarding fees
NoneMoving your billing to us costs nothing up front.
New and smaller practices
Monthly minimumFlexible plans for new and smaller practices: a set monthly minimum instead of a percentage.
Included in the full-service rate
- Eligibility and benefits checks (VOBs)
- Prior authorizations
- Coding and charge entry
- Claim scrubbing and electronic submission
- ERA and payment posting
- Denial management and appeals
- A/R and payer follow-up
- Patient billing calls and statements
- Monthly reports and a dedicated account manager
Your exact rate comes after the free A/R review, in writing, before anything starts. Credentialing is priced separately, per payer application: see credentialing prices.
What we need from you
Four things. That’s the whole handoff.
- Accurate charge capture
- Correct documentation
- Charges sent on time
- A contact for payer questions
The numbers you get every month
One short report, the same six numbers every month, so you can see whether your billing is healthy in a minute.
- CollectionsWhat came in, by payer and provider, against last month.
- Days in A/RHow long the average claim takes to pay, and which way it is moving.
- A/R over 90 daysThe share of open balances past 90 days, worked oldest first.
- Denial rateDenials by payer and reason code, and what we changed upstream.
- First-pass acceptanceThe share of claims accepted on the first submission.
- Net collection rateWhat you collected of what you were owed after contractual adjustments.
Switching from another biller or in-house billing
Most practices come to us with a backlog. We keep new claims going out every day while the old A/R is worked, so nothing is dropped in the handover.
- A free review of 90 days of A/R before you sign anything
- Payer, clearinghouse and ERA enrollments moved with a checklist
- Old A/R worked oldest and largest first while new claims go out daily
- Open claims from your previous biller tracked until they close
- Weekly calls until the first month is closed
We bill inside the EHR or practice management system you already use, including Epic, athenahealth, eClinicalWorks, AdvancedMD, Tebra (formerly Kareo), DrChrono, CareCloud and NextGen.
What a full in-house billing team costs
In-house billing is never one person. Someone checks insurance before each visit, gets authorizations, codes, posts payments, works denials and chases A/R. Price that team for your practice, next to our rate for all of it.

Pay is the U.S. median for each job in May 2025: billing and posting clerks $48,500 (BLS), medical secretaries and administrative assistants $45,930 (BLS) and medical records specialists, who include coders, $51,140 (BLS). Benefits other than paid leave were 22.5% of private employers’ compensation costs in June 2026 (BLS), so each yearly cost is pay divided by 0.775. The starting team uses one billing person for every two providers, a common in-house rule of thumb (Physicians Practice), plus front-end staff for insurance checks; change any count to match your office. Our estimate depends on your monthly collections and is confirmed in writing after the free A/R review. Credentialing is priced per payer.
What your practice gets
- Faster reimbursements and steadier cash flow
- Fewer denials and rejections
- Less admin load on your front desk
- Clean claim submissions
- Real-time reporting you can actually read
- A dedicated account manager who knows your practice
Specialty billing rules we already know
Most denials start upstream, in the code. If you want billing and coding reviewed together, ask for a combined assessment. It costs nothing and takes about a week.
Our full-service rate is 3% to 6% of your monthly collections. It covers the whole revenue cycle, end to end, not just claims. Where you fall depends on your volume: the more you collect, the lower your percentage. There are no setup or onboarding fees, new and smaller practices can choose a flexible monthly-minimum plan, and your rate is written into the agreement before we start.
The full revenue cycle: eligibility and benefits checks (VOBs), prior authorizations, coding and charge entry, claim submission, payment posting, denial management, A/R follow-up and patient billing calls, with monthly reports and a dedicated account manager. Credentialing is priced separately, per payer application.
One to two weeks in most cases. We set up secure access to your systems, move payer and clearinghouse enrollments, then run alongside your current process until claims are flowing, so there is no gap where claims sit unworked.
Yes. We work inside the system you already use, including Epic, athenahealth, eClinicalWorks, AdvancedMD, Tebra, DrChrono, CareCloud and NextGen. If yours isn’t on that list, ask; it is rarely a problem.
Yes. Solo physicians, nurse practitioners, therapists and small groups get the same team, reports and account manager as a large group, and because the price is a share of collections, you are not paying a large practice’s fixed fee.
No. If the service stops earning its keep, you should be able to leave. Your data, your payer enrollments and your reports stay yours.
Yes. We don’t list our clients on this site, but ask us and we’ll arrange it: with a current client’s permission, we share their contact details so you can hear how we work from them directly.

Send us your last 90 days of A/R.
We’ll tell you what’s collectable and what’s at risk, free.
